Which plan do I apply for?

Each applicant is eligible for only ONE plan at any given time. Applicants are required to apply for the plan that corresponds to his/her employment or membership status with an endorsing organization/association and age at time of application. Use the table below to determine the plan for which you are eligible.

PLEASE NOTE:

  • In order to apply, applicants must not be off on any type of leave (disability, maternity, etc.)
  • It is the applicant’s responsibility to notify Corsana of any changes in that may affect their health insurance coverage or billing (family status, province of residence, etc.)
  • Employees of endorsing organizations and/or members of an endorsing association are required to submit supporting documentation in order to be eligible for our plan.
  • If you do not belong to an endorsing organization//association, or are unsure, please contact our office to learn more about what coverage options we have available for you.
